All student-athletes participating in Amherst Christian Academy athletics are required to complete the Athletic Registration & Liability Form prior to participating in practices or competitions.

Student-athletes are also required to have had a physical examination within the past twelve (12) months. While ACA does not require a copy of the physical, families must provide the name of the physician or medical provider who performed the examination for verification purposes if needed.

Registration fees and all required forms should be submitted before the athlete’s first practice or competition. Payments may be made by check (payable to ACA), cash, or online through the Athletic Registration link. Families submitting payment by check or cash should mail or deliver their payment and completed forms to the Amherst Christian Academy office.

To ensure the safety of all participants and maintain an organized athletic program, student-athletes may not participate in practices or games until all required forms have been completed and registration fees have been received.
